New Patient Intake

If you need PEP (post-exposure prophylaxis) please call and press 0. Inform Staff you have had an exposure and need PEP. You should then proceed to the office. PEP must be administered within 72 hours of the exposure.

Dr. Donovan is only accepting patients with HIV, Hep C, Gender Dysphoria, or who are starting PrEP (pre-exposure prophylaxis)

Dr. Prelutsky, Dr. Tochtrop, and Nurse Practitioner Aida Trivic are not accepting new patients at this time.

New Patients will not be given a prescription for any controlled medications at their first appointment

​Patients requesting their new provider to take over the prescribing of a controlled medication from a previous provider MUST supply medical records showing how long they have been taking the medication for the request to be considered.

​Examples of controlled Medications

- Opioids (Morphine, Hydrocodone, Oxycodone, etc)

- Stimulants (Adderall, Vyvanse, Ritalin, etc),

- Benzodiazepines (Xanax, Lorazepam, Clonazepam, etc)

- Hypnotics (Ambien, Belsomra, etc)

Providers reserve the right to deny establishment of care for patients who decline to share previous medical records
