New Patient Intake

CURRENTLY, NEW PATIENT APPOINTMENTS ARE BOOKED OUT UNTIL March, 2025

New Patients will not be given a prescription for any controlled medications at their first appointment

Patients requesting their new provider to take over the prescribing of a controlled medication from a previous provider MUST supply medical records for the request to be considered.

Examples of controlled Medications

Opioids (Morphine, Hydrocodone, Oxycodone, etc)

Stimulants (Adderall, Vyvanse, Ritalin, etc),

Benzodiazepines (Xanax, Lorazepam, Clonazepam, etc)

Hypnotics (Ambien, Belsomra, etc)

Call the office and select the option for New Patients, or fill out the online Intake form.
