New Patient Intake

and select option 4 from the phone menu.
IF NEEDING PEP SELECT OPTION 3 FROM THE PHONE MENU.
New Patients will not be given a prescription for any controlled medications at their first appointment
Patients requesting their new provider to take over the prescribing of a controlled medication from a previous provider MUST supply medical records showing how long they have been taking the medication for the request to be considered.
Examples of controlled Medications
- Opioids (Morphine, Hydrocodone, Oxycodone, etc)
- Stimulants (Adderall, Vyvanse, Ritalin, etc),
- Benzodiazepines (Xanax, Lorazepam, Clonazepam, etc)
- Hypnotics (Ambien, Belsomra, etc)
Providers reserve the right to deny establishment of care for patients who decline to share previous medical records
